Care Transitions Intervention

Thanks to a grant from the State of Colorado, Home Care of the Grand Valley is please to announce the implementation of its Care Transitions Intervention (CTI) program.  The program is designed to encourage patients and families to assert a more active role in their health care during care transitions from hospital to home.  Along with he CTI program, Home Care of the Grand Valley welcomes two wonderful additions to our team; Sarah Johnson, MSW and Juanita Trujillo.  For additional information on the CTI program please call 298-3112.
